An hour is the midpoint, not a vow
The label's plain sentence is the one I repeat: for most men, 50 mg about one hour before. The same paragraph allows 30 minutes to 4 hours. People remember the hour and forget the wings. A man who swallows at 19:00 and expects a switch at 19:05 has set a test the tablet never sat.
Fasted, peak plasma lands between 30 and 120 minutes, median 60. That is why the hour is a midpoint, not a vow. Younger men sometimes report an earlier window. Others need the far end. One quiet try after a late meal does not retire 50 mg.
Maximum frequency is once per day. A second 50 mg at 23:00 because 20:00 'did nothing' is a stacked dose, not a second chance. If the first evening was a food problem, the second chance is another night with a clearer plate, not another tablet the same night.

Blue edges come from a retina enzyme
Sildenafil is only about 10-fold more potent at PDE5 than at PDE6. PDE6 sits in the phototransduction path of the retina. That gap is the usual explanation for a blue tint or a bright-light sensitivity for a few hours. It fades with the plasma. It is distinctive enough that people remember it and call it 'the Viagra light.'
Retinitis pigmentosa sits in the extra-caution pile. A man with a known retinal degeneration should not treat a 50 mg tablet as a cosmetic risk he can laugh off. That is a prescriber conversation with the eye history in the room.
Vardenafil can show a similar visual leftover in a few men. Tadalafil's signature leftover is more often a muscle ache. If the blue edge is the deal-breaker and the timing otherwise suits you, that is one reason to look at the other two clocks in the comparison, not a reason to double 50 mg and hope the color goes away.

25 mg and 100 mg live on the same card
This gallery hangs the 25-100 mg range because that is the whole Viagra ladder. 50 mg is the usual first rung, not a moral middle. 100 mg is the labelled ceiling, not a prize. 25 mg is a start for older men, hepatic impairment, severe renal impairment, alpha-blocker pairings, and several CYP3A4 stories, including a 25 mg cap in 48 hours with ritonavir.
With doxazosin already stable, the sildenafil start is 25 mg. A 100 mg swallow on top of an alpha-blocker was enough, in a small early series, to produce a long postural hypotensive evening. That is why the start is small when the other bottle is an alpha-blocker, even if a friend takes 50 mg with nothing else on board.
Do not split a 100 mg tablet to invent a 50 mg because the 50 mg pack was dear. Some tablets are not scored for that job, and the 'half' is a guess. Ask for the strength you mean. The timing rules do not change with milligrams. The side-effect odds do.

The 24-hour nitrate sentence is an unknown
Organic nitrates and nitrites in any form are contraindicated. So is riociguat. After a 100 mg dose in healthy men, plasma at 24 hours is about 2 ng/mL against a peak near 440 ng/mL. The label still says it is unknown whether a nitrate can be safely given at that hour. In men over 65, with hepatic impairment, with creatinine clearance under 30 mL/min, or with CYP3A4 inhibitors, that 24-hour leftover can be 3 to 8 times higher.
I treat that as a full hold, not a math problem you solve with a calculator. If chest pain starts, say sildenafil and the hour you swallowed it. Do not hand anyone a spray because 'it has been a while.' Tadalafil's leftover is a clearer 48-hour minimum. Sildenafil's leftover is an unknown wearing a low plasma number. Both are reasons to speak up.
Keep a line in a wallet or a phone medical ID on nights you take 50 mg. A partner should know the hour. Travel days are when people forget and an airport clinic reaches for the wrong drawer.

Wine and a flush are not one problem
Sildenafil 50 mg plus alcohol at 0.5 g/kg, enough to reach about 0.08 percent in a volunteer study, did not add a hypotensive punch to the alcohol. That is a narrow study sentence. It is not an invitation to drink hard and then swallow 100 mg. Alcohol still dulls sensation and judgment. A faint-feeling evening has more than one author.
Flush, stuffy nose, and headache are the class background. They track the evening, not the next morning, because the half-life is short. A stuffy nose is annoying. It is not a reason to add a decongestant cocktail without looking at blood pressure.
Priapism past 4 hours, sudden vision or hearing change, chest pain: stop and get urgent help. Those lines sit on every PDE5 page because people wait, hoping the clock will save them. The clock will not.

A high-fat dinner steals the peak

Viagra may be taken with or without food. That sentence is true and incomplete. With a high-fat meal, the rate of absorption falls: mean Tmax delay about 60 minutes, mean Cmax cut about 29 percent. Total exposure holds up better than the peak. The evening feels late and softer more often than it feels absent.
A cheese-heavy plate is the usual saboteur, not a glass of water. You do not need a medical fast. You need to stop treating a steak-and-sauce dinner as a fair first exam. A lighter plate, or a couple of hours after the heavy one, gives the 50 mg tablet the peak the trials assumed.
Mean absolute bioavailability is 41 percent (range 25-63). Volume of distribution is about 105 L. Protein binding is about 96 percent. CYP3A4 is the main clearance; CYP2C9 is the minor door. Grapefruit in large or repeated amounts is a CYP3A4 nuisance. A one-off segment is less of a story than a nightly habit.

Thirty minutes to four hours is printed
Most efficacy work that still gets quoted looked at about 60 minutes post dose. A RigiScan time-course study still saw an effect at 4 hours, weaker than at 2 hours. That fade matches a terminal half-life of about 4 hours for sildenafil and its N-desmethyl metabolite. Plan the evening you are in. Do not plan Saturday on a Friday 50 mg.
The 30-minute edge is real for some men and disappointing for others. I would rather a couple use the hour for a first walk than chase a stopwatch brag. Stimulation still has to happen. Sildenafil does not manufacture desire. A tablet swallowed in another room while nobody touches anyone will look like a failed drug.
If 50 mg at a fair hour is thin after several tries, the labelled step is 100 mg, not a tighter stopwatch. If 50 mg works and the head or the flush is too much, 25 mg is the labelled step down. Age over 65, cirrhosis, creatinine clearance under 30 mL/min, or a strong CYP3A4 inhibitor all argue for considering 25 mg first.

If the first two evenings disappoint
Before you retire 50 mg, ask three ugly questions. Was the plate heavy? Was there real stimulation? Was this only one night? A high-fat delay plus a nervous first try is a poor exam. Give two or three fair evenings, hour and plate aligned, before you ask for 100 mg or a different clock.
If the week cannot stand an hour and a plate, daily tadalafil 5 mg is the schedule that removes both. If the QT line is clean and sildenafil's food story is the only complaint, vardenafil is another evening, not a miracle. Those trades live in the three-clock review and the sildenafil profile.
Do not change the milligram from a dispatch. Age, liver, kidneys, alpha-blockers, and ritonavir already wrote some of those answers. A clinician with the list still has to say them out loud.
